Thank you for your inquiry regarding our services. The Department of Health, Developmental Disabilities Division provides and maintains an array of services for individuals with developmental disabilities and/or mental retardation. You will have an opportunity to learn more about our Branch and discuss the services that we offer as well as their availability during a scheduled interview. Additionally, if you choose, you may acquire assistance with the completion of your application during this time.
We believe in self-determination, that you have the ultimate say in the direction of your own life and services and we want to help you in any way possible.

If you are interested in applying, please call us Monday through Friday, during regular business hours (7:45am – 4:30pm). We have staff available to guide you through the intake process and answer any questions you might have.
2. Helpful Information for Your Consideration:
- Applications for services must be signed by the person with the developmental disability and/or their parent/guardian.
- The Developmental Disabilities Division has 30 working days from the date of receipt of the application to make a determination on eligibility for services or inform you if additional information is needed to make a determination.
3. Information We Need to Make a Determination:
To assist us in our determination on eligibility, we will need the following documentation:
- Psychological evaluations, if available.
Must be signed by a licensed psychologist and include I.Q. scores and available adaptive behavior assessment. - Physical examination, current within the year of application and any other medical summaries.
- Department of Education Conference reports/evaluations, if available.
- Other Reports and Assessments you feel would be helpful in our determination.
